(HealthDay News) -- Your doctor should always have your medical records on hand, but it''s a good idea to keep copies for you and your family at home. The Palo Alto Medical Foundation suggests keeping this information in a binder, including: diagnosed chronic conditions, known allergies, information vital in an emergency, and the name and phone number of your primary care doctor. An up-to-date medication list that includes doses and dates prescribed also is important. An immunization record, results from cancer screenings, records of major injuries and illnesses, and a list of major diseases in your family should be noted, as well.
